The Applicant applies to use:
There is a multi-facility discount of $50 when selecting (2) facilities.

The Applicant hereby agrees to pay the Board of CRYF the sum reflected based upon 1 day of use as shown above.  This amount shall be paid in full at the time this application and shall not be refunded for ANY reason except for the Board's denial of this application.  This agreement is only good for the day applied for and granted.  If there is ANY damage, or the building/grounds not cleaned after use, the applicant will be billed required repair and/or clean-up fees.

RULES

Certain general rules and regulations apply to the use of all facilities. The
Applicant agrees to comply at all times with these rules and regulations and all other
guidelines and policies adopted by the Board, of the CRYF, which apply to the use of
facilities. These general rules and regulations are as follows:

 

  1. The use or possession of firearms, other weapons, alcoholic beverages, or illegal drugs on this property are STRICTLY PROHIBITED.  The Applicant agrees to make required announcements regarding the prohibition of such items.
     

  2. NO Pets allowed at any time
     

  3. Loud or profane music/conduct on this property is prohibited
     

  4. The user should not violate and federal or state statutes or any local ordinance, including federal and state laws prohibiting discrimination.
     

  5. The facility shall be maintained in a clean & orderly manner and shall be left in the same or better condition when received.  User will be billed for any repair and/or cleaning fees if not.
     

  6. The named user shall provide, and will be responsible for complete supervision of all activities.
     

  7. The user accepts full responsibility for any personal injury or property damage, which may occur during the time period of use of the facility.
     

  8. Use of other facilities on the premises are NOT allowed unless user agrees to rent them as well. 
     

  9. The user shall fully indemnify and hold harmless the Board of the CRYF, its
    individual members, its agents and any other person connected to or related to
    this organization in any other way from any and all liability whatsoever arising
    from any personal injury or property damage as result out of the use of the
    facility.
     

  10. The Board of the CRYF or President may require any user of a facility to
    provide liability insurance in such amount as the Board of the CRYF or
    President deems necessary.
     

  11. The undersigned agrees that failure to implement and monitor the
    conditions of this application, especially, but not limited to the enforcement
    of Rule #1 above, may lead to termination of this agreement.

By signing below, the Applicant hereby certifies that he/she has full authority to submit this application on behalf of the Applicant and agrees to all fees, rules, and regulations.
